Opting out of the union will not cause you to lose hours, scheduled raises, health-insurance coverage or representation during the length of the contract and likely beyond.

Opting out of the union is simple. Fill out this form and click “submit.” Your completed opt-out form will be sent to the state to process.

Within 2-4 weeks you should no longer see “union dues“ or “nonmember fees” subtracted from your provider check.

If you change your mind, you can request to re-enroll in the union – although thousands of providers have been satisfied with their decision to opt out.
